Office Policies
We look forward to seeing you at your appointment. Please bring in the medical history form we mailed you or that you downloaded from the patient info page. Feel free to call us if you need help with the form. Bring any and all insurance cards with you also. If possible try to arrive a few minutes early so we can review your forms.
Please call the office for an updated list of insurances that we bill. We do not bill Maine Care.
If we do not bill your insurance, payment will be expected at the time of service. We will provide you a detailed receipt to submit to your insurance company for your reimbursement. Please make sure your primary care provider has completed a referral if needed for your insurance. You may have trouble getting reimbursed if not.
Please do not wear makeup or fragrance to your appointment. One interferes with careful examination and the second aggravates the numerous highly allergic individuals in our care.
